Manages front line staff members of TransitionManagement department and actively participates in supporting managementfunctions including staff development and evaluation of clinical practice.Develops and maintains standards of practice in accordance with evolvingpractice of case management, social work, and utilization management. Trendsand reports avoidable days, RLOS, readmission and other metrics to ensureoptimal performance. Effectively utilizes department Physician Advisors andMedical Directors (TMO & UM) to intervene when continued stay criteria arenot met. Serves as resource to facilitate complex cases and escalate concernsand feedback to department Director/Vice President.
Ensures the department's optimal performance among their team by engaging in effective problem solving with inter professional staff, clinicians, patients and families.
Responsible for team accountability to performance standards including quality, service, patient flow, communication and credo behavior. Interacts effectively with members of the health care team and ancillary departments to monitor and achieve quality patient outcomes.
Supports the delivery of quality patient care by holding staff accountable to established patient care and regulatory standards.

Master'sDegree (or equivalent experience) and 5 years relevant experience
Licensed Discipline Specific (Required) Registered Nurse or Licensed Master Social Worker or Licensed Clinical Social Worker or Licensed Advanced Practice Social Worker or Utilization Manager or Case Manager
Case management certification (ACM-RN, ACM- SW, or CCM) within one year

The Vice President of TMO is Marcia Colone, Ph.D. who has over 25 yearsof experience in care management.
