Care Manager I
Position Summary Provides care management and care coordination for adult and pediatric patients with mild to moderate illness, under minimal supervision, to optimize control of chronic conditions and prevent/minimize long term complications. Job Duties: Identifies the targeted population within practice site(s), per primary care provider referral and registry reports. Assesses the health care, educational, and psychosocial needs of the patient/family using standardized assessment tools such as depression screening, functionality, and health risk assessment.
