The Clinical Care Coordinator for Cross Sectional Interventional Radiology (CSIR) provides comprehensive nursing coordination and clinical support for patients undergoing diagnostic and therapeutic image-guided procedures across the care continuum. Working within a highly collaborative, multidisciplinary environment, this role supports Cross Sectional interventional radiologists, advanced practice providers, nursing staff, and referring providers in the management of complex acute and chronic conditions.
The CSIR Clinical Care Coordinator is responsible for facilitating efficient patient throughput by coordinating pre-procedure evaluation and communication, procedural readiness, and post-procedure follow-up for both inpatient and outpatient populations.
This role serves as a central point of contact for patients, families, and care teams, providing education, addressing clinical questions, and ensuring continuity of care. The coordinator identifies gaps in care, supports quality improvement initiatives, and partners with internal and external stakeholders to standardize processes and optimize patient outcomes.
The ideal candidate demonstrates strong clinical judgment, exceptional organizational and communication skills, excellent attendance, proficient computer skills and the ability to manage multiple priorities in a fast-paced, dynamic environment while maintaining a high standard of professional practice and patient-centered care.
Primary responsibilities include, but are not limited to:
Perform comprehensive telephone triage for pre and post procedure patients, including assessment of symptoms, complications, and appropriate escalation of care. Reviews clinic referrals and coordinates clinic scheduling
Responds to emails and in basket messages from physicians, advanced practice providers, clerks, nurses and referring providers, including new referrals
Review incoming requests for procedures, request outside records, and images and labs in a timely manner.
Coordination of care for clinic and procedure patients.
Post procedure follow-up with patients and to schedule follow-up clinic appointments.
Reviews upcoming procedures for appropriate scheduling and readiness (labs, orders, equipment, consents, imaging, etc.).? Attend weekly scheduling meetings.
Arranges for preauthorization of medications, complete and submit required insurance forms.
Arranges for prior authorization for requested imaging (MRI, CT, DVU).?
Serve as a point of contact for clerical and scheduling staff.
Point of contact for patient questions, pre-procedure education.
Obtains OSH imaging and uploads.
Participate in CSIR meetings as appropriate.
Provide intra-procedural nursing care including patient assessment, assist with procedural sedation, and document all aspects of patient care and education.
Other duties as assigned both clinical and non-clinical.
Current licensure as a Registered Nurse in the State of Michigan.
Bachelor of Science Degree in Nursing, OR associate's degree/Diploma in Nursing along with a Master Degree in Nursing.
Minimum 5 years of current (within last 2 years) RN experience providing care to adult procedural (CPU, MPU, IR, OR) or Pre-Op/PACU.
Two years of recent (within the previous 5 years) experience in a clinical leadership role such as charge nurse, preceptor or similar role demonstrating organizational, prioritization and multi-tasking skills.
Effective communication and problem-solving skills demonstrated by employment references and feedback from others and performance evaluations.
Self-directed and motivated to work independently and collaboratively with other health care providers as demonstrated by employment references and feedback from others and performance evaluations.
Ability to manage multiple tasks simultaneously.
ACLS, BLS and CAC Certifications
Previous work performance demonstrates self-direction, autonomy, and flexibility.
Strong leadership skills as evidenced by previous evaluation documentation.
Previous work performance demonstrating excellent critical thinking and problem-solving skills.
Demonstrated expert communication skills as evidenced by interviews, references, and performance evaluations.
Participation in unit activities that demonstrate the ability to work collaboratively with and positively contribute to a team approach to patient care.
Demonstrated high standards of personal and professional conduct and attendance as evidenced by interviews, references, and performance evaluations.
